ABOUT:
CMC was established more than 50 years ago, when volunteers from the San Joaquin Medical Society saw a need to provide healthcare to farmworkers and their families. Today, that volunteer effort has grown into Community Medical Centers (CMC), a non-profit network of neighborhood health centers. CMC is a vital resource for underserved residents in San Joaquin, Solano, and Yolo counties. The uninsured, rural, and migrant families, the working poor, and the homeless, rely on CMC for care and support.

Services include primary and preventive care, reproductive health care, pediatrics, behavioral health, substance use services, health education and WIC (Women, Infants & Children). Each patient is cared for by a team including a provider, medical assistant, and receptionist. An extended team may include a nurse, behavioral health clinician, case manager, health educator, or specialty provider, depending on the needs of the patient. This approach encourages comprehensive, integrated, whole person care. And no matter who is on the care team, the most important member is always the patient.
