My Appeal
Many people equate the word hospice with dying, but at Home Health & Hospice Care, hospice is so much more. When medical care can no longer offer a cure, hospice focuses on providing patients with comfort, dignity and quality of life at the end of life. Our team of healthcare professionals takes pride in providing compassionate, attentive care to patients and their families as the journey of life comes to an end. We provide hospice care in patient's homes, in local nursing homes, and at our Community Hospice House in Merrimack, NH.
We do receive compensation from Medicare, Medicaid, and some insurance companies, but this is most often not enough to cover someone’s care. 100% of the funds received stay within the communities we serve and helps to support the Community Hospice House; an end-of-life care facility with 16 state-of-the-art patient suites.

About the Organization: Home Health & Hospice Care has been a provider of home care and hospice services since 1883, making us the region’s most experienced Visiting Nurse Association. Serving 25 communities in southern New Hampshire including Manchester, Nashua, Derry, Salem, Merrimack and beyond, we provide traditional home care, inclusive of nursing, rehabilitation, therapies, medical social work, and licensed nursing assistants. Our Hospice team cares for patients in their homes, in nursing facilities, and in the Community Hospice House, which is owned and operated by Home Health & Hospice Care. We pride ourselves on having superior outcomes, cutting-edge technology and being leaders in disease management.
